Per CISD Policy physicals for the 2026 - 2027 school year must be dated after April 1, 2026
At CISD we use Rank One Sport to manage your athletes medical information. This is a secure site that is HIPAA and FERPA Compliant. Here you will complete your students pre-participation forms. Please fill out all forms truthfully and completely. If you need assistance please feel free to reach out to one of the Staff Athletic Trainers.
2026 - 2027 Physical Day
Who? Athletes, Cheer, Drill Team, HS Marching Band, ROTC, Student Manager, Student Filmers, Student Trainers
When? April 15, 2026
Where? Oak Ridge HS Athletic Foyer (27330 Oak Ridge School Rd, Conroe, Texas, 77385)
Cost? $25 cash at the door, $26 Online Prepay
**Please come with a completed 26-27 Physical and Medical History Form signed by the parent and student in BLUE or BLACK ink**
Current HS Students (Grades 9-11)
Current JH Students (7-8)
Current 6th Graders
Bussed over during athletics
Please see coach on what time to arrive on campus.
8th Grade students will need to be picked up from ORHS Athletics Foyer
Anytime between 8:00 AM - 5:00 PM
You must be in line to be seen by 5:00 PM, Doors will close at 5:00 PM
